International Mosaic Committee

In CISV we often love to complain about our many forms, which some people find complicated and annoying to fill out. But of course, forms are absolutely essential to help things run as smoothly as possible. And we are happy about our forms for two reasons:
- There are only two forms, and you will only have to fill out one of each per program - there doesn't have to be one for every participant like some of the other programs. It's up to your staff and chapter as to whether you need to provide health or insurance forms to all of the participants. As far as we're concerned, the below forms are the two that you NEED to understand and use to run a program.
- Our forms are not all that complicated to use, and - for the Proposal/Report form in particular - helps you to plan and design your project so that it is in line with the program requirements. It may look complicated, but believe us - it'll make your job so much easier!

- This is for chapters to request to host Mosaic projects.
- The completed form must be submitted to the International Office by June 1st each year, to request any projects which might take place between December of that year and November of the following year.
- The form also allows chapters to select a “Priority Period” - a period prior to approval of the Project Proposal, in which the IMC offers full support to the project staff and coordinator in the planning stages.

- This is the official Mosaic project form – it is to be used both before the project begins (to outline the proposal) and after it is complete (to evaluate and report against the original goals and objectives).
- The proposal should be completed using this form and should be sent to the IMC. The committee will then decide whether or not to approve the project based on this form. Once this form gets approval your activity is considered an official Mosaic project.
- After your project is complete you should also use the form to report on its outcomes by comparing the proposed project with how the actual activity turned out. You should send this report to the IMC within one month of the project finishing.
